Nas \u00faltimas d\u00e9cadas, os avan\u00e7os terap\u00eauticos proporcionaram aumento significativo nas taxas de remiss\u00e3o completa. Entretanto, a persist\u00eancia de pequenas quantidades de c\u00e9lulas leuc\u00eamicas ap\u00f3s o tratamento inicial, denominada Doen\u00e7a Residual M\u00ednima (DRM), tem se consolidado como um dos principais fatores progn\u00f3sticos na condu\u00e7\u00e3o cl\u00ednica da doen\u00e7a.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Nesse cen\u00e1rio, o laborat\u00f3rio assume papel fundamental, uma vez que a detec\u00e7\u00e3o da DRM exige m\u00e9todos de alta sensibilidade e interpreta\u00e7\u00e3o t\u00e9cnica especializada. Assim, a monitoriza\u00e7\u00e3o laboratorial cont\u00ednua permite avaliar a resposta terap\u00eautica e orientar decis\u00f5es cl\u00ednicas de forma mais precisa.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full is-resized\"><img fetchpriority=\"high\" decoding=\"async\" width=\"900\" height=\"600\" src=\"https:\/\/atlasemhematologia.com.br\/blog\/wp-content\/uploads\/2026\/04\/image-7.png\" alt=\"\" class=\"wp-image-17486\" style=\"aspect-ratio:1.5000146485805526;width:494px;height:auto\" srcset=\"https:\/\/atlasemhematologia.com.br\/blog\/wp-content\/uploads\/2026\/04\/image-7.png 900w, https:\/\/atlasemhematologia.com.br\/blog\/wp-content\/uploads\/2026\/04\/image-7-300x200.png 300w, https:\/\/atlasemhematologia.com.br\/blog\/wp-content\/uploads\/2026\/04\/image-7-768x512.png 768w\" sizes=\"(max-width: 900px) 100vw, 900px\" \/><\/figure>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Conceito e import\u00e2ncia progn\u00f3stica da Doen\u00e7a Residual M\u00ednima<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">A Doen\u00e7a Residual M\u00ednima refere-se \u00e0 presen\u00e7a de uma pequena quantidade de c\u00e9lulas leuc\u00eamicas remanescentes ap\u00f3s o tratamento, que n\u00e3o s\u00e3o detect\u00e1veis por m\u00e9todos convencionais, como a avalia\u00e7\u00e3o morfol\u00f3gica por microscopia \u00f3ptica. Apesar de invis\u00edveis em exames rotineiros, essas c\u00e9lulas mant\u00eam potencial proliferativo e podem levar \u00e0 reca\u00edda da doen\u00e7a.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Al\u00e9m disso, a detec\u00e7\u00e3o da DRM possui elevado valor progn\u00f3stico. Pacientes que apresentam DRM positiva ap\u00f3s a indu\u00e7\u00e3o da remiss\u00e3o apresentam maior risco de recidiva e, consequentemente, pior desfecho cl\u00ednico quando comparados \u00e0queles com DRM negativa. Dessa forma, a avalia\u00e7\u00e3o da DRM tornou-se um marcador essencial para estratifica\u00e7\u00e3o de risco e planejamento terap\u00eautico na LLA.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>M\u00e9todos laboratoriais para detec\u00e7\u00e3o da DRM<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">A identifica\u00e7\u00e3o da Doen\u00e7a Residual M\u00ednima requer t\u00e9cnicas laboratoriais com elevada sensibilidade anal\u00edtica. Entre os m\u00e9todos dispon\u00edveis, destacam-se a citometria de fluxo multiparam\u00e9trica, a rea\u00e7\u00e3o em cadeia da polimerase quantitativa (qPCR) e, mais recentemente, o sequenciamento de nova gera\u00e7\u00e3o (NGS).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">No contexto brasileiro, a citometria de fluxo multiparam\u00e9trica representa o m\u00e9todo mais acess\u00edvel e amplamente utilizado para avalia\u00e7\u00e3o da DRM. Esse m\u00e9todo permite identificar assincronismos de matura\u00e7\u00e3o e combina\u00e7\u00f5es espec\u00edficas de ant\u00edgenos de superf\u00edcie presentes exclusivamente nas c\u00e9lulas leuc\u00eamicas.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Adicionalmente, a t\u00e9cnica de PCR possibilita a detec\u00e7\u00e3o de rearranjos g\u00eanicos espec\u00edficos, como aqueles relacionados aos genes de imunoglobulinas e receptores de c\u00e9lulas T, bem como transcritos quim\u00e9ricos resultantes de transloca\u00e7\u00f5es cromoss\u00f4micas. Essa avalia\u00e7\u00e3o cont\u00ednua contribui para identificar precocemente pacientes com risco elevado de reca\u00edda, antes mesmo do aparecimento de manifesta\u00e7\u00f5es cl\u00ednicas evidentes.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Al\u00e9m disso, os resultados laboratoriais relacionados \u00e0 DRM auxiliam na tomada de decis\u00f5es terap\u00eauticas. Pacientes com persist\u00eancia de DRM podem necessitar de intensifica\u00e7\u00e3o do tratamento, uso de terapias-alvo espec\u00edficas ou indica\u00e7\u00e3o de transplante de c\u00e9lulas-tronco hematopo\u00e9ticas.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Consequentemente, o laborat\u00f3rio n\u00e3o atua apenas no diagn\u00f3stico inicial da LLA, mas tamb\u00e9m no monitoramento da efic\u00e1cia terap\u00eautica e na avalia\u00e7\u00e3o progn\u00f3stica, desempenhando papel estrat\u00e9gico na assist\u00eancia ao paciente onco-hematol\u00f3gico.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Refer\u00eancias:<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">IKOMA\u2010COLTURATO, Maura RV et al.
