Embora essas c\u00e9lulas normalmente permane\u00e7am restritas \u00e0 medula \u00f3ssea, sua presen\u00e7a no sangue perif\u00e9rico pode refletir altera\u00e7\u00f5es importantes da resposta imunol\u00f3gica ou dist\u00farbios hematol\u00f3gicos. Al\u00e9m disso, participam de rea\u00e7\u00f5es inflamat\u00f3rias cr\u00f4nicas por meio da libera\u00e7\u00e3o de mediadores capazes de promover dano tecidual. A eosinofilia, portanto, pode estar associada a doen\u00e7as al\u00e9rgicas, helmint\u00edases, s\u00edndromes inflamat\u00f3rias persistentes e neoplasias hematol\u00f3gicas.<\/p>\n\n\n\n<p>Por outro lado, os precursores eosinof\u00edlicos, como miel\u00f3citos e metamiel\u00f3citos, n\u00e3o circulam habitualmente no sangue perif\u00e9rico. Sua detec\u00e7\u00e3o sugere est\u00edmulo medular intenso ou desorganiza\u00e7\u00e3o do processo de matura\u00e7\u00e3o. Esse achado pode ocorrer em infec\u00e7\u00f5es graves, rea\u00e7\u00f5es leucemoides, uso de fatores estimuladores de col\u00f4nias e em doen\u00e7as hematol\u00f3gicas espec\u00edficas, como a leucemia eosinof\u00edlica cr\u00f4nica e as s\u00edndromes mielodispl\u00e1sicas. Dessa forma, a avalia\u00e7\u00e3o conjunta da contagem e da morfologia dos eosin\u00f3filos \u00e9 indispens\u00e1vel para investiga\u00e7\u00e3o diagn\u00f3stica.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img decoding=\"async\" width=\"545\" height=\"213\" src=\"https:\/\/atlasemhematologia.com.br\/blog\/wp-content\/uploads\/2026\/01\/image-3.png\" alt=\"\" class=\"wp-image-17210\" srcset=\"https:\/\/atlasemhematologia.com.br\/blog\/wp-content\/uploads\/2026\/01\/image-3.png 545w, https:\/\/atlasemhematologia.com.br\/blog\/wp-content\/uploads\/2026\/01\/image-3-300x117.png 300w\" sizes=\"(max-width: 545px) 100vw, 545px\" \/><\/figure>\n\n\n\n<p><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Caracter\u00edsticas morfol\u00f3gicas e matura\u00e7\u00e3o dos bas\u00f3filos<\/strong><\/h3>\n\n\n\n<p>Os bas\u00f3filos s\u00e3o leuc\u00f3citos granuloc\u00edticos derivados da linhagem mieloide e sua diferencia\u00e7\u00e3o ocorre na medula \u00f3ssea sob a influ\u00eancia de citocinas espec\u00edficas, com destaque para a interleucina-3. Essas c\u00e9lulas atuam como mediadoras da resposta inflamat\u00f3ria e das rea\u00e7\u00f5es de hipersensibilidade imediata, liberando histamina, heparina e citocinas pr\u00f3-inflamat\u00f3rias ap\u00f3s ativa\u00e7\u00e3o por receptores de IgE.<\/p>\n\n\n\n<p>O processo de matura\u00e7\u00e3o dos bas\u00f3filos ocorre em est\u00e1gios sequenciais, desde o mieloblasto at\u00e9 o bas\u00f3filo segmentado. Nos est\u00e1gios iniciais, as caracter\u00edsticas morfol\u00f3gicas se assemelham \u00e0s de outras linhagens granuloc\u00edticas, o que dificulta sua distin\u00e7\u00e3o. A partir do miel\u00f3cito bas\u00f3filo, contudo, surgem gr\u00e2nulos metacrom\u00e1ticos no citoplasma, permitindo a identifica\u00e7\u00e3o microsc\u00f3pica da linhagem. Com a progress\u00e3o da matura\u00e7\u00e3o, observa-se maior condensa\u00e7\u00e3o da cromatina nuclear, altera\u00e7\u00f5es na forma do n\u00facleo e aumento da granula\u00e7\u00e3o citoplasm\u00e1tica, culminando no bas\u00f3filo maduro.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img decoding=\"async\" width=\"294\" height=\"294\" src=\"https:\/\/atlasemhematologia.com.br\/blog\/wp-content\/uploads\/2026\/01\/image-2.png\" alt=\"\" class=\"wp-image-17209\" srcset=\"https:\/\/atlasemhematologia.com.br\/blog\/wp-content\/uploads\/2026\/01\/image-2.png 294w, https:\/\/atlasemhematologia.com.br\/blog\/wp-content\/uploads\/2026\/01\/image-2-80x80.png 80w\" sizes=\"(max-width: 294px) 100vw, 294px\" \/><\/figure>\n\n\n\n<p><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Identifica\u00e7\u00e3o, libera\u00e7\u00e3o e interpreta\u00e7\u00e3o laboratorial<\/strong><\/h3>\n\n\n\n<p>A presen\u00e7a de eosin\u00f3filos e bas\u00f3filos imaturos no sangue perif\u00e9rico deve ser avaliada com cautela e reportada de acordo com boas pr\u00e1ticas laboratoriais. Tradicionalmente, os campos destinados \u00e0 contagem de granul\u00f3citos imaturos s\u00e3o voltados para a linhagem neutrof\u00edlica. Assim, eosin\u00f3filos e bas\u00f3filos imaturos devem ser inclu\u00eddos na contagem total de suas respectivas c\u00e9lulas maduras. Ex: \u201cPresen\u00e7a 2% de miel\u00f3citos bas\u00f3filos, acima inclu\u00eddos na contagem de bas\u00f3filos\u201d.<\/p>\n\n\n\n<p>Entretanto, \u00e9 fundamental que esses achados sejam descritos de forma clara na se\u00e7\u00e3o de observa\u00e7\u00f5es do laudo. A especifica\u00e7\u00e3o do tipo celular identificado e de sua quantidade fornece informa\u00e7\u00f5es adicionais valiosas ao cl\u00ednico.
