De fato, a hipersegmenta\u00e7\u00e3o de neutr\u00f3filos \u00e9 um dos achados morfol\u00f3gicos mais espec\u00edficos, estando associada a dist\u00farbios da matura\u00e7\u00e3o celular e altera\u00e7\u00f5es na s\u00edntese de DNA.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Conceito e Crit\u00e9rios Laboratoriais<\/strong><\/h3>\n\n\n\n<p>A princ\u00edpio, os neutr\u00f3filos s\u00e3o granul\u00f3citos polimorfonucleares com n\u00facleo segmentado normalmente em 3 a 4 l\u00f3bulos conectados por finos filamentos cromat\u00ednicos. No entanto, a hipersegmenta\u00e7\u00e3o \u00e9 definida pela presen\u00e7a de neutr\u00f3filos com 5 ou mais segmentos nucleares. Dessa forma, entre os crit\u00e9rios frequentes para caracteriza\u00e7\u00e3o incluem-se:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Neutr\u00f3filos com \u2265 5 segmentos nucleares;<br><\/li>\n\n\n\n<li>Presen\u00e7a de neutr\u00f3filos com \u2265 6 l\u00f3bulos.<\/li>\n<\/ul>\n\n\n\n<p>Assim, no exame laboratorial, a identifica\u00e7\u00e3o precisa da hipersegmenta\u00e7\u00e3o requer an\u00e1lise manual em esfrega\u00e7os sangu\u00edneos corados pelo m\u00e9todo Romanowsky (Wright ou Giemsa). Portanto, \u00e9 necess\u00e1rio analisar pelo menos 100 neutr\u00f3filos para avalia\u00e7\u00e3o quantitativa da frequ\u00eancia de c\u00e9lulas hipersegmentadas.<\/p>\n\n\n\n<p>Embora os analisadores hematol\u00f3gicos modernos ofere\u00e7am dados quantitativos e \u00edndices morfol\u00f3gicos, a hipersegmenta\u00e7\u00e3o n\u00e3o \u00e9 detectada diretamente por m\u00e9todos automatizados. Neutr\u00f3filo hipersegmentado<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Significado Cl\u00ednico da Hipersegmenta\u00e7\u00e3o de Neutr\u00f3filos<\/strong><\/h3>\n\n\n\n<p><strong>Defici\u00eancias Nutricionais: Vitamina B12 e \u00c1cido F\u00f3lico: <\/strong>A principal associa\u00e7\u00e3o cl\u00ednica da hipersegmenta\u00e7\u00e3o neutrof\u00edlica est\u00e1 relacionada \u00e0 defici\u00eancia de vitamina B12 e\/ou \u00e1cido f\u00f3lico.&nbsp;<\/p>\n\n\n\n<p>Dessa maneira, essa defici\u00eancia provoca inibi\u00e7\u00e3o da s\u00edntese de DNA, causando atraso na matura\u00e7\u00e3o nuclear dos neutr\u00f3filos e consequente segmenta\u00e7\u00e3o exagerada. Al\u00e9m disso, essa altera\u00e7\u00e3o \u00e9 caracter\u00edstica da anemia megalobl\u00e1stica, frequentemente acompanhada de macrocitose, anisocitose e presen\u00e7a de c\u00e9lulas megalobl\u00e1sticas na medula \u00f3ssea.<\/p>\n\n\n\n<p><strong>Anemia Megalobl\u00e1stica: <\/strong>No hemograma, a hipersegmenta\u00e7\u00e3o neutrof\u00edlica representa um dos achados morfol\u00f3gicos cl\u00e1ssicos da anemia megalobl\u00e1stica, que se manifesta tamb\u00e9m com altera\u00e7\u00f5es quantitativas e qualitativas dos eritr\u00f3citos e leuc\u00f3citos. Portanto, a identifica\u00e7\u00e3o da hipersegmenta\u00e7\u00e3o auxilia no diagn\u00f3stico diferencial com outras anemias macro ou normoc\u00edticas.<\/p>\n\n\n\n<p><strong>Neoplasias Mielodispl\u00e1sicas (NMD): <\/strong>Nas NMD, a hipersegmenta\u00e7\u00e3o pode ocorrer junto com outras displasias neutrof\u00edlicas, tais como hipogranula\u00e7\u00e3o e anomalias nucleares, refletindo falhas na hematopoiese. Assim, reconhecer essa altera\u00e7\u00e3o morfol\u00f3gica \u00e9 importante para o diagn\u00f3stico e monitoramento dessas doen\u00e7as hematol\u00f3gicas.<\/p>\n\n\n\n<p><strong>Outras Causas<\/strong><strong><br><\/strong> Al\u00e9m das causas j\u00e1 mencionadas, a hipersegmenta\u00e7\u00e3o pode estar associada a:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Uso de medicamentos que interferem na s\u00edntese de DNA (ex.: metotrexato, hidroxiureia);<br><\/li>\n\n\n\n<li>Condi\u00e7\u00f5es de estresse hematopoi\u00e9tico;<br><\/li>\n\n\n\n<li>Idade avan\u00e7ada, em que ocorre uma discreta hipersegmenta\u00e7\u00e3o fisiol\u00f3gica;<br><\/li>\n\n\n\n<li>Algumas infec\u00e7\u00f5es cr\u00f4nicas.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Par\u00e2metros Hematol\u00f3gicos Associados<\/strong><\/h3>\n\n\n\n<p>&nbsp;Al\u00e9m da hipersegmenta\u00e7\u00e3o, o hemograma geralmente evidencia:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Aumento do volume corpuscular m\u00e9dio (VCM), caracterizando macrocitose;<br><\/li>\n\n\n\n<li>Aumento do RDW, indicando anisocitose;<br><\/li>\n\n\n\n<li>Pancitopenia ou leucopenia vari\u00e1vel.<br><\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Desafios Diagn\u00f3sticos e Relev\u00e2ncia Laboratorial<\/strong> <strong>da Hipersegmenta\u00e7\u00e3o de Neutr\u00f3filos<\/strong><\/h3>\n\n\n\n<p>&nbsp;A hipersegmenta\u00e7\u00e3o pode ser subdiagnosticada quando o exame manual n\u00e3o \u00e9 realizado cuidadosamente ou quando \u00e9 interpretada fora do contexto cl\u00ednico.
